Background: There are few studies that compare outcomes between primary and revision tympanoplasty. Considering controversial studies, we decided to determine
whether or not the revision surgery act as a prognostic factor for graft success rate.

Objectives: This study was designed with the aim of comparing the outcome of primary tympanoplasty with revision tympanoplasty based on closure of TM
perforation and hearing improvement.

Materials and Methods: This study was a prospective one which was performed in Shahid Dastgheib Hospital, Shiraz, Iran. The study included CSOM patients who
underwent primary or revision tympanoplasty without mastoidectomy between January 2012 and November 2014. Postoperatively, graft success rate and the
hearing results were the main criteria for measuring outcome.

Result: The results showed that there are significant differences between two groups in graft success rate (GSR) (P=0.001). SRT was 24.9±13.1 in revision group
and was 23.1±9.2 in primary group. But the percentage of ABG less and equal to 20 db in primary group was 83.8% and it was 76% in revision group (P=0.26).

Conclusion: In conclusion, graft success rate is significantly better in primary tympanoplasty but primary does not have any superiority than revision in hearing outcomes.
