Rational surgical techniques in managing cholesteatoma.

The pathogenesis of cholesteatoma is still complex and there are many factor s contributing to its development. The main challenge that comes with its treatment modalities is the residual and recurrent disease problem.
To overcome this many techniques have been defined.
By using intact canal wall technique we achieve better functional results however with this technique, the development of residual and recurrent disease seriously threatens the success of the operation.
Contrarily the open techniques coincide with the functional outcome of the surgery in regard to hearing and quality of life.
Planning the surgery due to location and behavior of the cholesteatoma by also considering the contributing factors on its pathogenesis will serve for obtaining
functional outcomes with less morbid surgeries.
Surgical planning for Cholesteatoma should be based on knowledge for the mechanisms of cholesteatoma formation, a thorough preoperative evaluation and individualization of the surgical technique.
