Comparison of Tympanoplasty Outcomes In Unilateral Versus Bilateral Chronic Otitis Media In Khalili Hospital From 2009-2014

Background: chronic otitis media is a recurrent infection of the middle ear in the presence of a tympanic membrane perforation . infection is a principal cause of tympanic membrane perforation . the goals of the treatment of chronic otitis media (COM) are to stop otorrhea , heal of tympanic membrane , eradicate current infection , prevent complications , and prevent recurrence .
Objectives: the goal of this study is comparison of tympanoplasty outcomes in unilateral and bilateral COM who underwent surgery between march 2009 and march 2014 .
Materials and methods: we retrospectively reviewed recorded medical file of 168 patient ( 112 female and 56 male ) with unilateral chronic otitis media and bilateral chronic otitis media who underwent tympanoplasty in shiraz (southern iran) khalili hospital between march 2009 and march 2014 . graft success rate and hearing result were the main criteria for measuring outcome .
Result: the results showed that there are not significant differences between two groups in graft success rate (p=0.4) . SDS was 97.01+/-4.53 in unilateral COM patients and 95.3+/-8.62 in bilateral COM patients . SRT was 28.08+/-13.5 in unilateral COM and 30.44+/-15.4 in bilateral COM patients . but the percentage of air-bone gap less and equal 20 dB in unilateral COM patients was 75.89% and it was 75% in bilateral COM patients . (p=0.206)
Conclusion: in conclusion , graft success is not significantly better in unilateral COM patients , the unilateral COM patients have better hearing outcomes after tympanoplasty than bilateral COM patients but the differences is not significant . (p=0.206)
