Introduction: cerebellopontine angle (CPA) lesions are the predominant skull base neoplasms that affect the posterior fossa The most common symptoms of acoustic neuroma is Sensorineural hearing loss (SNHL), tinnitus, dysequilibrium, and facial hypesthesia. There are different strategies for treatment of CPA tumors.

Materials and Methods: In this comparative case series 22 cases of CPA tumor was compared during 6 years from 2014 to 2019. We observed them for 1 to 6 years. We compare outcomes, complications and other features of these different approaches in this study.

Results: A total 22 patients including 18 females and 4 male were included in the study. The mean age was 49years. Our planning for 15 cases was translabyrinthine surgery and for 7 cases because of no hearing loss and normal neurologic signs and symptoms were observed. Four people after the surgery had facial paresis that recovered during six months after it in tow of them.

Conclusion: In CPA angle lesions planning should be more than surgery strategies. Close observation and stereotactic radiation should be included in management modality.
