Instruction course of Tympanoplasty : Technical point in difficult cases

Tympanoplasty is the most common surgery in the field of otology. The main goals of tympanoplasty are elimination of infection, closure of middle ear cleft and restoration of the ear function. To accomplish these goals, it is imperative that the otologic surgeon exercise clinical judgement in terms of selection of patients and surgical approach.
There are enormous techniques for performing tympanoplasty and being familiar with these approaches would help us in the different types of drum perforations. Most of surgeons think tympanoplasty is an easy approach but it is not a true fact. We may face many situations during this kind of surgery that requires enough knowledge about the techniques. Knowing the history of tympanoplasty and the embryology and anatomy of tympanic membrane is essential for starting this surgery. Nowadays using cartilage is very popular in this surgery and there are many concepts about when and where and how we should put cartilage in the middle ear.
Recently some otologists use endoscope instead of microscope for approach to the middle ear. In this instruction course in seconds PONS congress I would like to give enough information from basic principles of tympanoplasty and discuss about some difficult situations.
